Insurance

At this time, Longevity+MD™ does not participate with insurance companies, which means our patients will probably be reimbursed on an out-of-network basis for most visits. Longevity+MD™ will provide the records and the itemized bill, that you would have to file to your insurance. Each insurance agency has different rules and we advise you check with your insurance carrier regarding reimbursement rules. Patients can, and many do, use their Flexible Spending Accounts (FSA) or Health Savings Accounts (HSA) to pay for their medical services.

Cost

We provide concierge service without an annual fee, with a Dr. Gibson’s hourly rate of $250. All prior patients of Dr. Gibson will get a 30% discount for the first 6 months. Program cost varies, schedule free consultation about the services ( link to contact form).
For all visits, payment is due in full at the time of service. We do not require a retainer, but scheduling an appointment requires an active credit card be placed on file so that you may be billed for the services provided, including any cancellation fees if applicable.

We understand chronic illnesses and lifestyle changes take time, patience, and trial and error to achieve improvement. Please note that our practice is small and is designed to provide chronic disease management, not acute care.

If you have a new symptom(s), you should be evaluated by your primary care provider to be sure that you do not have a new, unrelated problem.

If you have worsening and/or acute symptoms you are concerned about, you should call 911 or be evaluated by the nearest urgent care facility or emergency department.

If you are having an exacerbation of your illness, please do not delay your care and contact your primary care provider or schedule an appointment in an urgent care facility Please let us know about the exacerbation via patient portal messaging or on the phone and we will schedule the first available appointment.

Most medication changes and initiation of any new medication will be made during office visits after an appropriate evaluation and not over the phone, the portal or by asking our office staff to ask Dr. Gibson if she can prescribe a medication.

Programs

New Patient Visit

Includes 90 minutes time with Dr. Gibson to review outside records, your history, symptoms, personalized goals, screenings, assessments and perform a detailed physical examination.

She also tries to fit in as much education as possible during this visit and usually makes initial recommendations for management strategies.

This does not include the follow up visit, which is recommended for most patients (see below).

Synthesis of the complex information with a detailed consultation report including assessments and recommendations.

Exceptionally complex patients may require more time to address all issues; this will need to be scheduled as a separate visit.

Medical records:

  • You are encouraged to organize and provide the most pertinent medical records in order to improve the efficiency, cost, and effectiveness of your assessments to avoid redundancy and reduce cost.

  • Provide the results of pertinent tests, if available. If not available, Dr. Gibson will order the screenings, but it will delay your follow up consultation.

Basic Necessary Tests:

Laboratory tests ( within the last 3-6 months): CBC, CMP, HGBA1C, lipid panel, TSH, HCRP

Radiological and other tests and pertinent consultation notes from other providers ( within the last 6-12 months).

Cancer screenings:

  • colonoscopy, HIT, Cologuard ( within last 3-5 years)

  • Mammogram (within last 2 years)

  • Pap smear ( within last 3-5 years)

  • PSA (for men within last 12 months)

Cardiovascular Screening

  • Stress test

  • Calcium score CT

  • Echocardiogram

  • Others

DEXA scan (within last 2 years)

-Weed out pages of your records that contain no useful information or are duplicates.

The patient who provides a random stack of 1000 pages with multiple copies of the same results or a CD with years of provider follow-up notes unrelated to your current issues will result in a less valuable visit than the patient who brings an organized binder with Labs, Radiology studies, other studies, and consultations.

- Organizing your thoughts ahead of time to allow a focused review of your story is also helpful. For example, summarizing in 10-15 minutes all the medical issues you may have faced in your childhood years is more helpful than spending 15 minutes detailing one experience that happened when you were 5 years old.

Follow Up Visit

Includes 90 minutes of Dr. Gibson’s time.

Face-to-faceTime: 60 minutes when Dr. Gibson will go with you through all your assessments, recommendations, and detailed plan.

If more than 60 minutes is needed, the extra time will be charged at the current hourly rate.

This includes Dr. Gibson’s time to review the initial consultation note, and analysis of your assessments, labs and tests
